But the science is increasingly telling a different story, and it matters for anyone thinking about what they put in their body.

The Difference Between "Sugar-Free" and "Low Sugar"

These two terms are not the same:

Sugar-free means a product contains no sugar (sucrose), but it almost always uses chemical or synthetic sweeteners to replace the sweetness ingredients like sucralose, aspartame, acesulfame-K, or sugar alcohols like erythritol and xylitol.

Low sugar means a product contains a reduced amount of natural sugar compared to standard alternatives. The sweetness comes primarily from naturally occurring carbohydrates. In our case, maltose and glucose syrup are derived from natural starch sources, plus 7% real fruit juice.

What the Science Says About Artificial Sweeteners

1. Gut microbiome disruption

A 2024 study published in PMC examining five common artificial sweeteners, Acesulfame K, Saccharin, Sucralose, Rebaudioside A (stevia), and Xylitol, found that synthetic sweeteners like Sucralose and Saccharin significantly reduced gut microbial diversity over 35 days. Sucralose, in particular, was found to enrich pathogenic bacterial families, including Enterobacteriaceae, which are associated with inflammation and gastrointestinal disease.

By contrast, the same study found that non-synthetic sweeteners caused less disruption and promoted beneficial bacterial taxa like Lachnospiraceae.

2. Glycaemic disruption despite zero calories

One of the most counterintuitive findings in sweetener research is that some zero-calorie sweeteners can impair glycaemic control, the very thing they were designed to help. A study cited in Medscape found that noncaloric artificial sweeteners, including saccharin and sucralose, contributed to intestinal dysbiosis and poor glycaemic control in short-term trials with healthy participants.

In other words: reaching for a sugar-free product to manage blood sugar can, in some individuals, worsen blood sugar control through an indirect gut pathway.

3. Sucralose and the microbiome

Sucralose (commercially known as Splenda), combined with maltodextrin, has been linked to a significant decrease in beneficial intestinal microbes in animal studies. Sucralose alone has been shown to decrease intestinal bacteria and amino acid synthesis and to increase inflammation markers.

4. Erythritol and digestive tolerance

Erythritol, one of the most commonly used sugar alcohols in "healthy" snacks, can cause digestive discomfort including bloating, cramping, and diarrhea when consumed above individual tolerance thresholds. While it has a better gut microbiome profile than some synthetic sweeteners, it is not without side effects.

 


 

Why Natural Sugar in Moderation Is Different

Natural sugars, when consumed in moderate amounts, are processed by the body through well-understood metabolic pathways that have evolved over millions of years:

Maltose (from natural starch) is a disaccharide broken down by the enzyme maltase in the small intestine, producing glucose for cellular energy.

Glucose syrup derived from natural sources provides a slow, moderate energy release without the sudden spikes associated with high-fructose corn syrup.

Fruit juice sugars, fructose and glucose in fruit, are accompanied by the fibre, vitamins, and polyphenols of the fruit itself, which moderate their absorption and add nutritional value.

The body knows how to handle natural sugars. The enzymes, gut bacteria, and metabolic systems required to process them have been developed through evolutionary history. There is no such co-evolution with sucralose, saccharin, or acesulfame-K, which is one reason their effects on the microbiome are less predictable.

 


 

What About People with Diabetes?

This is the most common context in which "sugar-free" products are sought, and the most important one to get right.

The common assumption is: less sugar = better for diabetes. But glycaemic impact is not determined only by sugar content. It is determined by:

1. Total carbohydrate content, including sugar alcohols, which still contributes to carbohydrates

2. The speed of absorption is influenced by the food matrix (fibre, fat, protein present)

3. Gut microbiome health, which strongly affects insulin sensitivity and glucose tolerance
